APPLICATION STRATEGIES: LESSONS FROM THE PAST...

Let's begin with a quote by Arie De Gues to give ourselves a good context why this post is important:

“The ability to learn faster than your competitors may be the only sustainable competitive advantage.”

Which is pretty reflective on what Aristotle had once said,

''The secret of business is to know something that nobody else knows''.



Having said that, back to our topic :

1. Apply on time
: For God's sake ! and that means September to very early-October 2007 if you are aiming for the 2008 Residency match! Start your deadline clocks today ! And gather all required stuff on time - Time goes fast while we are busy, and Sept. 2007 will be here faster than you think ! (Yes - you can apply
only with Step 1 Scores too and mention the Step2CK date on your application). And of course, read about the technical reason to apply on day one - if you are not ready to.

2. USMLE Steps Planning: Plan Steps such that you have Step 1 and Step 2 CK IN HAND at the time of applying. Grant about 4-weeks to the ECFMG slow-pokes for your scores - schedule accordingly ! This assures the maximum number of interviews. If you are applying on a single score like ONLY step 1 or ONLY Step 2 CK - interviews are much lesser !

3. USCE - US Clinical Experience / Research Experience ! Try hard to get this, coz it now seems to be the most important application booster after your scores ! A lower score with US clinical experience may get ranked high by program directors than someone with high 90s but no clinical experience. Match 2006 certainly has proved that USCE is very important to program directors ! As an additional proof of its rising importance, check out this IMG requirements page put up by the Pediatric Residency Program at University California at San Francisco.

Be advised that in terms of importance / preference for Residency Applications:

Clerkships, Electives, Externships > Observerships > Short-Term Clinical Research


Dedicated Long term Clinical Research is gaining a lot of importance as far as residency admissions are concerned and a good number of publications go a long way helping you out. Read about advantages of Clinical research for the residency seeking IMG.


More dedicated to explanation of the various types of USCE in a separate post.
Externships or Sub-Internships, Mini-Residencies, and Clerkships / Electives are considered true USCE since they give you hands-on clinical experience, whereas Observerships ( a.k.a 'Shadowing') only give you a chance to be a passive observer without being allowed to touch the patient. Start contacting hospitals where you can go and try to fix them early - so that you can specify that on your ERAS application that its arranged. Even you arrange it later, you can always call up the programs that you have interviews with or interviewed with later, so that they rank you high in mid-February. Again , USCE has proved important in the 2006 match , with many 95-plus scorers going unmatched while 80-scores with USCE matching ! So take note ...

4. US Letters of Recommendations (LORs): Try to get atleast 2 LORs from US doctors / Clinical Researchers - your application will get considered way high ! Read more about them at LOR basics for Residency.

5.ECFMG Document Verification: If you medical school is slow about verifying your documents from ECFMG - get on their asses and get it done before applying too - trust me, it helps to be ECFMG certified atleast before you interview. Don't forget to have your Dean's letter or the MSPE ready by July-August.

6.Take the Step 2 CS (CSA) Early: If you have not already, my personal recommendation is book the earliest day for Step 2 CS Today ! It takes only 15 days to study well for it and now you can take it even before Step 1 (Preferrable, given the crazy delays in score reporting in the later half of the year). Last year, many Step 2 CS results were delayed and people could not participate in the match ! To see the earliest date available at this very moment at the 5 CSA centers in USA - Click Here! . Some programs like the Internal Medicine Residency Program at the Penn State's Milton S. Hershey Medical Center would like to see Step 2 CS results even before you apply to their program, hence, its in your best interests to have as many Step Scores ready before you apply so that you can be eligible for as many programs as you can be. Even ECFMG issued a notice urging candidates to book a date before the end of July to ensure a date for the year. 7. Start early on your Personal Statement : Don't make a sloppy last-minute job out of it. For every specialty you apply for, it is advisable to have two formats for your Personal Statements - one for Community Programs and the other for University-based programs. 8. About Step 3 and H1B Visa: If you wish to be considered for an H1B sponsorship, Step 3 must be completed ideally before you even go for the interview - but thats painting a very very ideal picture. The second best would be to have your results
before the programs submit their candidate ranking to NRMP. Will be writing more on Step 3 on a separate dedicated post.

9. Targetted Applying approach: When the time to apply arrives, you will be busy calling up programs to know visa policies and other policies such as minimum scores, USCE requirements, Reco requirements and so on. Many of you will call up those hundreds of programs and decide where to apply. Thats cool - and I advocate that approach - But, in addition, it helps to make it targetted basing whom you call depending on which programs actually gave out H1 Visas in the past ! I have begun compiling a list of IMG-friendly residency Programs at http://www.USMLEtoMD.com/imgfriendly based on collected information from pals and will be completed soon.

10. Speeden up your USMLE prep: Agreed that these studies get pretty boring - so get your a study partner - Enlist yourself for free at www.USMLEbuddies.com. Besides study partner listings - more new innovative resources will keep getting added - that are otherwise hard to find - such as Interview Experience Submissions Database , which has about 270 First Hand Interview Experiences by residency applicants and gives a lot of info about Individual programs like Fellowship chances, Visas offered, Prematches given or not, etc. Other resources will are scattered here and there - so grab the diamonds ;-)


11. Interview Performance: Again , the year 2005-06 has proven how much this matters ! In year 2005, A well-known IMG-Friendly university program called about 200 candidates for interviews, but only ranked about 50-60 candidates based on interview performances and assurances from candidates! So - it's not just your personality that matters on an interview, but also how much you can assure the program director that you are genuinely interested in the program.

12. No Gaps on the Resume ! This is particularly more important for the IMGs taking the USMLEs is not viewed as an excuse for a 'time-off' and sitting home. So try your best to keep something going on like some nominal research or a part-time clinical experience. Of course, this won't be a concern for those involved in some academic program or medical school.

13. Professional Help: A little bit of that does not matter - of course as long as you are not paying too much ! That's why quiet a few friends of mine and other people on the forum have been using these neat books which are not too expensive either:

I neededsome advice.... i m taking my step in end of July...CS in august and Step 2 CK in Nov...which completes my certification in Jan 2007 ...but i want to aprticipate in 2007 residency match....can you suggest some tips as how can I accelerate my process and be ready...Do I have any cahnces to match or post match from the above mentioned schedule...I dont want a J1 or H1 sponsorship ...have U.S experience and LORS.

Hi ...the fact that you wont need a sponsporship certainly is a big plus. If there is no way you can make ur prep for step 1 and step2 faster - a great on step 1 will certainly get u noticed - besides your no-visa requirement. Number of Interviews will be lesser than what you would get if you had both scores in hand by Sept. Also , chances to match depend on which specialty you are applying for. IM is high-competition. Peds / FP may land you more interviews. Thus - if you simply have to match, you may consider applying to more specialties than just one.

Accelerating ...basically would mean getting exams done earlier in your case - but do not screw up scores in the process :-) A Good step 1 score is important for you to get more interviews.

i am waiting for my step 2 ck score(will come in a week or so.) and i'll have my CS result before september 1.
i do not have LORs, GC etc...
applying early is the only positive i have. do u think with such a score and no backing of LORs, its worth that i put in so much time monew and energy? will i get any interview calls with this. i want pediatrics.

Hi...thnx for the kind comments :-) Pediatrics has less competition than IM - beside applying early, a strong Statement of Purpose is another positive you can have ...be ready for J1s too and also, in your case, maybe you just wanna take up any pre-match offer you get...in case interviews are not too many. Hello...thnx for dropping by. For Step 2 CK, you can assess ur readiness based on NBME performance & USMLEworld performance. You can shoot an email to USMLEworld guys after u solve blocks..and ask them how much does that indicate on the real test - they are usually helpful.

You may try this year - and that depends on what specialty you want..Psychiatry, Pediatrics and FP may be easier on low scores as compared to IM, Neurology, Radio etc... CS wont more than 15-20 days of prep. I would suggest that you take ur chances this year..at the same time after ur exams, get observerships and stuff to strengther ur resume..

Hi again..Lets look at possible pathways once you enter FP :

1. Finish FP - enter primary care (jobs in hospitals, group practices OR start own clinic 0 many jobs around)
2. FInish Fp - do fellowships in Geriatrics, Sports Medicine, Obg-gyn, etc.
3. Switch from FP to IM in second year - by applyint to match again and using FP 1st year as US Clinical Experience :-)
4. Arrange with Internal Medicine in the same hospital as ur FP residency - to offer you double certification - in IM and FP ..takes 4 years and am not sure if ALL places allow that.

Yes...you will either have an H1 or J1 visa for the residency

Reply 1 : Dean's Letter or MPSE is needed...however, if you dont have one, ECFMG site says that if you notify them, they will place a nominal MPSE in-lieu of the absent one (do look up ECFMG.org)

Hi Saggi...please dont let ur confidence sag in face of exams - u need all the good spirits u can gather ! Which specialty are you interested in ? With ur Major, something like pathology would be a great fit..Pathologists have a lot of respect and excellent pays in the US.

You may be late for IM programs..but you may get lots of interviews for FP , depending on how many you apply too...

Whoa - looks like i have been neglecting questions - sorry folks..let's answer some :

1. Saggi - Psychiatry remains much easier to get in than IM - yet H1 sponsoring programs are far less as compared to IM - now, with ur low scores, to increase chances, also apply to those combined Family-Practice / Psychiatry programs and try to get done with your Step 3 as soon as you can & then with a great Personal statement ..am sure you shud get interview calls :-)

2. Oscar - Medical diploma will be needed for ur ecfmg certicification - so if u get certified before February of the Match year you are aiming for - that should not be a prob !

3. Neuro for u shud not be tough..it has been been pretty easy so far when it was in the SF-match, lets see how it goes with NRMP this year. Competition however also is from first year IM residents who can easily get into Neuro..since the first year for Neuro is a prelim IM year..

4. Guy asking bout Interviews - yea u can still apply thru ERAS - u dont certification for ERAS application (Do read the link of ECFMG Certificate)

5. Applying for both should not decrease chances - mention your devotion to IM strongly on ur Personal Statement. If called for interview and they ask why u applied for both, let them know that u did so coz you were ready to take up even a prelim year if the program felt your experience was lacking ..and that u simple HAVE to do IM :-)

Answers :

1. Patho : Yes, do email ALL the programs you applied to and did not hear back from ..to know wut the deal is. Patho is getting very competitive - not only coz AMGs like it now, but also coz the program was made a 4-year instead of 5-year in 2006. The last year, the programs increased positions by 1 or 2 to made adjustments for reduced residents as the 5th year residents left..and now again this year, the seats have been reduced in most places, since they now are adjusted with more residents..(thats wut i heard from a patho guy). So do call, and you may wanna apply to more commuinty based programs too.

2. Step 1 and 2 Ck Failure: Think back on what your weak subjects were and give yourself more time this time. Start with weak subjects, though ur mind will hate doing that, and make sure you get a good score on NBME exams before u take the real test - those are good indicators. If you get good scores and also try to get some observerships / externships, that will surely enable u back in the peds game. It will be a long term plan for u - so u will need to prepare urself for the long-term wait and consider all costs.

Give 4 to 5 months for each step ..and in addition plan such that u have step 3 ready too, to improve chances.

Answer 1: yea, you can come back - try to have a email print or even beter - a letter from the physician granting you the observership / externship experience

Answer 2: In my opinion CS prep does not matter much on whether you took step 1 / 2 CK or not - its more of a general clinical grasp. Maybe doing your final year clinical internship will make you more prepared for the CS and hence would be better to take after it immediately the final year and do look up first aid for CS to get an idea on what to emphasize during that final year :-) g/l

Answer 1 = with your citizen status and US clinical experiences, you may have a good chance, especially if you a fresh graduate. But do spread the risks and apply to many programs and not just in your home state.

Answer 2 = You do have very good chances in pediatrics pal - with all that great UK experience and the fact that Pediatrics is not yet as competitive as IM . g/l

I have been doing a lot of info gathering mainly over the net, esp from sites like yours. I wanted to have your take on how Indian manage their stay in the US, esp during interview season. I do have family there who I can stay with for a few days. But, they have their own routines and I can't impose on them for nearly 2 months. How have the others before me made it? Any advice, ideas, opinions will be highly appreciated.
Also, concerning the Externship that everyone seems to say is extremely important for gaining USCE, are there many Indians who actually go from India to do it? Coz those things are ridiculously expensive. I know it's an investment for a chance to get in to a great program and earn more, but it's not easy to save up that much on a pitiful doc's pay. Not all parents can afford to pitch in either. Please give me some insight on this.

Staying with friends, relatives and friends of friends and occasionally hotels is what the Indian people I know do. For the Interviews, most programs will arrange for the stay the previous night. I know a guy who actually made an excel spreadsheet of all the possible contacts in USA he could garner through friends, friends of friends. etc. - which turned out a pretty big list which surprised him too ;-) Most people are helpful and would not mind you piling over for 2-3 days, since you will be moving from place to place during the interviews atleast.

yes Externships are expensive for those,most people who do it are already based / working in the USA or have r family that supports that with a loan. Observerships are more commonly done and are usually arranged informally by writing to faculty listed on the websites. Usually those are free. My take it, for those who cannot afford the expensive externships offered = 1. at least go ballistic on applying to individual programs and hospitals for observerships, 2. Yes - get the scores right, coz' that will be ur trump card. 3. Do as much as you can to buff up your resume in your home country..like research, publications, some experience with the big names in your country and 4. Always have backups in case residency does not work out for that year....

After all, who gets in and who does not get in a residency program, does not follow any fixed rules or patterns, its more of a generalization ...and in such cases, we can only do what's in our hands - give it the best we can in our capacity and leave the rest to providence :-)

I will do my best to get my highest possible score, and most likely I will do an Externship, too.[fortunately, my parents are supportive] I am told that Observerships do not count as much of hands-on CE. The only plus point is that they are free[but then again, you can see why]

'Volunteering' is a broad perspective term and cover anything from supplying meals to being a clinical assistant. So find out before hand, the exact nature of work you will be involved in., whether you will have a chance to interact with physicians, etc. Many hospitals have need-based duties assigned to volunteers.

If very clinical, they maybe you can take it and keep applying for more experiences while doing so.

Digital Doc! Thanks for these wonderful posts. I really need some help from you. I had applied to both psych and med programs and due to wrong judgement ended up matching at a psych program. In one of your replies above you mention that it is possible to apply for Int Med PGY2 with a PGY1 in FP.
a.)Can I do the same with a PGY1 in Psych?
b.) Are PGY2 spots in Int medicine frequent?

Answer 1 = Yes, it's best to still apply early in September 2007 for the 2008 match, even if only have 1 or two scores while applying. If you expect a Green card before the start date of 2008 residency, then you could mention EAD on your Visa status - technically it would be wrong, but since you will not be needing visa sponsorship, it is functionally correct - and that itself will get you more calls than otherwise.


Answer 2: Congrats for match, more than half the IMGs do not even get a match :-) Now about open positions in PGY2, the best resource is FindaResident.com. Click on the summary of open positions link you see on the home page, and you should be able to see the available positions anytime. yes psych PGY1 will be almost like Internal medicine and should be accepted by programs.

Answer 3: Yes, current touch with clinal experience helps - but US / Canada / UK experience will be valued more , and even in India, clinical experience in an academic setting where you have a senior physician to evaluate you, will matter more. Not saying you should discontinue, but concentrate on good and timely scores for the ERAS application first

Answer 1: Now that you asked, I shall deal with PGY1 - PGY2 stuff in a separate post. Internal Medicine is more of inpatient stuff - which family medicine will deal more outpatient primary care, screening procedures and usually a good amount of ob-gyn.

There are a fewer fellowship options after Family medicine. I have mentioned them on this post on family medicine salaries - scroll down the end of the post.



Answer 2: Usually Both steps are considered - at times if your step 1 score is very good, with a good resume, then even without Step 2 Ck scores, interviews are possible, but people with both step 1 and step 2 CK scores are considered first.


Answer 3: A Missed attempt is not reported on the transcript. But you lose all the money :-(

A medical degree is not required for the interview - but you certainly need it to get ECFMG certification and hence to be on the programs rank order list. You can call up ECFMG about using a provisional degree certificate for medical document verification

i just got an interview invitation from IM st. barnabas.......seems like they r have come out 1st in giving IVs

they have said in their email that they got 2000 applications last year......and this year they have got 1800 in 1st week only

Answer 1: The road map idea is good and shall work on it. You don't need to be it the USA to start the visa process and can have the paperwork mailed to you, and the stamping done in your home country visa counselate.

Answer 2: The preference is usually for fresh medical grads with high scores and US experiences - but many programs also prefer candidates with Post graduate experiences - especially those with research publications. So you could research on residency programs and make a list of programs that conduct endocrine research and even have endocrine / diabetes fellowships - write to both residency and fellowship directors in such places expressing your interest. Some will reply, some wont..and if you are lucky, some may even offer direct fellowships based on the demand-supply scene. OR you might land up US research experience opportunity too.

Basically what I am suggesting is that a targeted applying approach will work better for you rather than blindly applying to programs from circulating IMG-friendly lists.
