Which Countries do most IMGs in US come from ?

A 2006 American Medical Association Discussion paper on IMGs called the "International Medical Graduates in the U.S. Workforce" did a pretty good study on the IMG distributions and other workforce data.


Here are some interesting facts that the paper presents :

1. As of 2006, IMGs came from 127 Countries !

2. IMGs constituted 185,234 physicians out of a total of 794,893 in USA

3. 44% IMGs worked in the Area of Primary Care

4. And here are the Top-10 Countries that most IMGs came from =

i. India : 24%
ii. Phillipines : 10.6%
iii. Mexico : 6.7%
iv. Pakistan: 5.7%
v. Dominican Republic: 3.8%
vi. Russia: 2.9%
vii. Grenada: 2.8%
ix. Egypt: 2.6%
x. South Korea: 2.5%


Interestingly Iran makes about 2.3% of the IMG population (12th on the Top-20 IMG countries list) despite Iran being a restricted area for taking the USMLEs or applying for exam visas. I had a reader ask me how could an Iranian IMG go about USMLEs and Residencies - I have yet to work on finding that out. I would appreciate any comments on that issue.

Another interesting thing is that while the Billion-plus populated India makes the most IMGs, its Billion-plus populated neighbor, China, ranks 11th. Pathology has a pretty good Chinese IMG population, but overall their volumes have stayed low on the clinical side.

With a big physician shortage projected in the United States, IMGs will continue to play a big role in America health care arena.

English. No wonder Chinese take up Pathology! (No offence towards them).

I am sure they are taking up that too seriously, so Indian IMGs face competition there in future, watch out :-)

I was in the UK and knew a couple of Iranian IMG's who were giving the USMLE. Both of them were born in the UK and then their parents shifted back to Iran.

Thanks for your comments guys. Yes the chinese are hardworking and fast learners of English. I had a chance to be around two Chinese IMGs and I was impressed by their hardworking nature and determination to learn English and not let it hinder them.

Hi ,I am from Iran and I am doing my residency in texas.I born and grew up there. that information is absoultly wrong there is no restriction for taking usmle exams for iranian at all!!
if we want to take the exam we have to go to another country like turkey ,.... for taking exam.
there is no Amrican center in Iran for taking the exam,
when I applied for residency I recieved more than 15 IV.
only problem is getting visa to come to USA , it is hard ,that's it.

Thank you for your comment - What you say is exactly what I said - that there are restrictions for taking the USMLE exams in Iran - I did not mean the Iranians :-)

Two Questions for you:

1. Is Turkey the most common destination for Iranian IMGs to take the USMLE ?
2. For US visa for Step 2 CS / Step 3, do Iran IMGs apply from outside Iran or within ?

Hi. I'm an Iraq citizen currently studying Medicine in Egypt. I did 2 years at Baghdad University-School of Medicine before I had to transfer here for security reasons and also for the fact that Iraqi Medical Schools may loose their ECFMG accreditation due to the current circumstances in Iraq.

And I can tell you that almost every medical student in the Arab World (including myself) is planning on working and doing postgraduate training in one of the so-called Anglosphere countries. With US, UK, Canada being the most wanted respectively. Not only for economic incentives but also for the high regard of medical training in these countries here.

The only problem that's facing us currently is visa restrictions which is essentially due to the latest events relating to terrorism and what's not.
For example, I personally know an Iraqi guy who graduated from an ECFMG-accredited Iraqi Medical School. He did his USMLE 1 & 2CK in Amman, Jordan. His scores were so high that they had his photo framed and is now displayed in the AMIDEAST center (America-Mideast Educational and Training Services) in Jordan.
After all his hard work, he was denied a US visa three times.

Another Iraqi guy that I know has gone all the way and got a US visit visa, did the Step 2 CS, got an offer from a hospital in Texas after the residency matching and then he was denied a work visa and later forced to leave before his visa expired !!!

All this has made me shift my plans to UK, Australia, or Canada which have more lax visa regulations even though it's been my all-time dream to visit and live the US.
Or at least let's hope by the time I graduate after 3 years, it becomes easier for Iraqis to get a US visa.

Dr. Digitaldoc, thanks for your words. And thank you for the valuable information you're providing on this blog.
Believe me it's the genuine hope of everybody here in the Arab World that the actions of the ignorant extremist minority wouldn't effect our image as ordinary people. Nevertheless it did effect it to a great extent whether we like it or not.


Now if I may, I have some questions regarding IMGs trying to work in the US that I hope you could answer:

1-Unquestionably it's way harder for IMGs to get into competitive specialties than for US graduates.
My question is, let's suppose one got a residency spot ,did extremely well, and got a good recommendation. Will s/he be able to compete with US graduates when applying to fellowships? In other words , will the fact that they are IMGs haunt them for the rest of their medical careers regardless of how well they do in the residency?

2-Do the salaries differ considerably between IMGs and US graduates? Also, will the fact that they are not US graduates effect them pay-wise all through their medical careers (even after finishing residencies and fellowships)?

3-After doing the residency and expiration of the HIb visa, is it to be expected that one would not be granted a new visa in order to do the fellowship?

4-Lastly, when shall someone be eligible to get certified from the American College of a specific specialty (e.g. FACS, FACC)? In other words, how many years does s/he have to work in the US after finishing a fellowship in a particular specialty before being a fellow?

I hope I'm not bothering you with these long questions!!

Several editorials in the AustralianDoctor publication have alluded that the Indian Government has been utilizing its immense population as human capital by overproducing more "white-collar" professional graduates to swarm developed countries and dominate those industries there because the remittance from its transient but stupendous diaspora is extremely lucrative. Is that true?
