THINKING 2008 RESIDENCY MATCH ? BOY U GOTTA START NOW !

Lately I have been power-cycling a lot on my 21 Speed Mongoose Bike into the dense Oak forest trails to take advantage of the lovely American fall season. The Fall happens so regularly, each year...just like the phases of moon keep repeating...or the way you keep coming back to same "Do Re Mi' notes octave after octave...Cyclical events, just like our residency applications, year after year. Yet, each residency-match year has its own distinct flavor and dynamics. Early planning has remained a vital credential for highly competitive programs and I would like this post to make hopeful IMGs aware of deadlines - both Technical and Logical. This year the Internal Medicine Program at Abington Memorial Hospital received 700 IMG applications with scores above 90 on both steps WITHIN the first week of September ! Competition is intense ...So read on...and plan while you still have time ...

Sometimes, while planning, it's best to work our way backwards from our ultimate target, so that you not only know what and when to do stuff, but also WHY to do it. So know the deadlines and act on them, otherwise its all futile, as Soren Kierkegaard puts it :

"It is perfectly true, as philosophers say, that life must be understood backward, but they forget the other proposition, that it must be lived forward."





Lol...I see that I have built up enough drama for this post ;-) Hmmm, So lets suppose you were planning on starting a Residency in 2008 ( All Residencies begin on the 1st of July of the year)



1. The 'Match' is a computer algorithm owned by the National Resident Matching Program (NRMP) that will occur around Mid-March 2008. The algorithm will 'match' program preferences on the list submitted by candidates with interviewed candidate preferences submitted by the residency programs. These lists are called "Rank Order Lists" (ROL).



2. When are these ROLS Submitted to NRMP ? That will be around the Middle of February 2008



3. Now, in order to have your name added on that ROL of the residency programs that you interviewed at, before the middle of February 2008, this is what program directors want to see from you :

a. Be 'ECFMG Certified' by then (i.e. Cleared Step 1, Step 2 CK, CS and have Medical documents/credentials verified with medical school)

b. Have passed Step 3 (if you need an H1b Visa) - please note that you can apply for a Step 3 Permit only after you have RECEIVED your ECFMG Certificate - which takes two weeks to arrive after passing all the previous 3 steps - Step 1, 2 CK and 2 CS.



4. Technically, applying to residencies and then participating the match is handled by different organizations - ERAS does Residency applications while NRMP does the matching part , so by December 2007, do not forget to Register yourself for the match - costs 40$

Hmmm, so far so good, Lets continue backwards further...



5. The peak Interview season will be Oct-Nov-Dec 2007 , while some programs (especially the ones that offer Pre-Matches) will send out Interview offers to applicants as early as September 2007, often on a first-come-first-serve basis. In order to be called for Interviews, obviously you should finish applying to programs through the ERAS (Electronic Residency Application Service) before the Interview season. To maximize your chances for interviews and get an edge over the competition, it is highly recommended that you apply EARLY (ideally 1st of September 2007) and your application should be complete (with Step 1 , Step 2 CK scores reported and Step 2 CS Passed by then)



6. Now, let's give a thought to what credentials you should have completed BEFORE applying on September 1st 2007 ?

a. Step 1 , Step 2 CK and Step 2 Passed and scores reported (Step 1 and Step 2 scores take 4 weeks for reporting and CS scores take about 8-10 weeks)

b. US-clinical Experience (US Clinical Electives, Observerships or Externships) - either completed or ongoing or secured [ so that you can mention on your application while applying itself]

c. All documents scanned by ECFMG and uploaded to the ERAS virtual mail-box so that they are ready for programs to download by the time you apply - as explained in point 7


( At this I point I would to state again that this timeline is an IDEAL scenario so stay ahead of competition - it does NOT mean that you cannot technically apply without all scores in hand - you can apply even on a single score... but people with all the above ready will always have that advantage..so do aim for this! Also read about ECFMG Certification)



7. Next, in order to be able to apply on 1st September 2007 deadline these are the things that must be taken of BEFORE 1st Sept. 2007:

a. you should have finished sending required documents [MSPE , LORs ,Copy of Medical school Transcript] to ECFMG-ERAS for scanning, by the middle of August - ECFMG says it will take about 10 business days for finishing scanning everything and making them ready to be attached to your ERAS applications - but this year, the time taken was more ! So the earlier you are , the better. Let me say something more ....actually, all your documents should REACH ECFMG by the middle of August. Hence, based on which part of the world you are applying from, you will need to have ready and mail them even earlier that middle of August 2007.

b. Completed filling up all the details the online Common Application Form (CAF) on the ERAS website and completed writing Personal Statements for all specialties. Note here that Personal Statements are often underestimated on the time that they will take to prepare and finalize. I would say, to a get a completely satisfactory Personal Statements, it takes 7-10 days of drafting, editing, tweaking, polishing and feedback from friends :-)

c. Completed compiling a list of programs to apply - i.e. your own 'IMG-Friendly List' of Programs - again it's best to prepare this by individually calling/emailing programs from the various IMG-friendly Programs listed Here. This takes a lot of time too. Thus, all this pre-application prep is easily gonna make August 2007 your busiest month - so dont plan a vacation then ;-) !




8. Next, in order to have ECFMG accept your documents and to be granted access to the ERAS website, you first need to have an AAMC ID - and that is obtained ONLY after you purchase the electronic 'ERAS token' online through your OASIS account on the ECFMG.org. And when will ERAS tokens for the 2008 Match be available ? First week of July 2007 ! So grab it as soon it is available (for 75$) and get the ball rolling - coz even the ERAS Token takes anywhere between 5-10 days to be granted after you have made the purchase :-)


So if you wish to have an ideal situation as described below....the time to act is now !

yea..if u take Step 2 CK by July 2006, ur results shud be well on time before September 2007 - and do register for CS right now ! dates get filled up very fast..and often people lose out on the match because CS was not taken on time...
Secondly - a little off-topic..you "take" an exam .."giving" an exam is wut the exam-supervisor or USMLE.org does..though in countries like India and Pakistan, local languages often use "give" to indicate the american "take" :-) i used to make the same mistake myself ;-)

Take it atleast 2 months before the date u want the results by...and dates will go away very soon from December onwards..plus it takes time to get the permit .and since the permit you get for CS is valid for one whole year, it does not hurt to apply for the permit early..

Hi..yeah, that sounds like a good plan. Step 3 will take you a month or 1.5 months, if you start studying immediately after your Step 2 CK !. Dont wait for Step 3 results , go ahead and apply with the other 3 scores on Sept. 1st !

Hi Kalai, it all depends on how comfortable and how fresh you are with ur knowledge - some people need less time - but it is highly recommened to test your scores on the NBME before you take either step 1 or step 2 CK, to assess where u stand :-) CS in March is great.

TO Pradeep: The "Permit" I am referring to is the Scheduling Permit for Step 2 CS exam which basically gives you permission to choose an exam date after you have applied for the test and ur application has been approved. Visas have now become easy for Step 2 CS - all you need to show is the Permit letter for visa, which will be provided to you by ECFMG - u will most likely get a one-month CS Visa, but some people even get 10-year multiple entry visas ...the luck factor ;-)

Thats's a good schedule - but apply for ur Step 2 CS permit early so that u can get a date u want - the permit is valid for a year , unline step 1 and 2CK permits that are valid for 3 months

1. to the ortho - aspirer : Ortho remains very tough even for AMGs to get in - high scores, US experience and LORs will go a long way. u shud also apply to G. surgery as a backup - suppose u do land up in G.surg, u can work ur way into the attached ortho program thru patience and contacts and leveraging the G.surg Residency as a prelim year for subsequent ortho residency. Also Green card and Citizenship status helps improving chances for Ortho ..tough, but with diligence, hardwork, tact and patience, it would not be totally impossible

Answer 2. Apply for CS by Dec this year - the permit is valid for a year , and then take the suitable date. Yes u can take the CS even before Step 1 ! (CSprotocol.blogspot.com)

Observerships - start looking up and applying now..it takes time to find one ..and try to get as many months as possible - I would asvise a minimum of 3 months, though most docs will give u LORs even after a month :-)

was jus checking 2007 cs reporting schedule. for examinees testing between may20 to july 14, the reporting start date is on aug 15th.. i plan to gve my cs sometime june mid..giving step 2 in april end.by the dates,,,seems like i won't be getting my results on time to apply for sep 1st..how do i plan?by reporting start date they mean they will only start posting on aug 15th rit? and is it necessary to get the ecfmg certificate to be able to apply for match or is the result for cs sufficient?

Thanks for ur kind comments pal. As per experiences of some people, it seems like, for IM, some programs prefer ECFMG certification for granting interviews. But I think great Step 1 and Step 2 CK scoress or lots of US Clinical Experience should help circumvent that. To stay ahead in the competition, if possible, try to finish as much as u can before Sept. 1st or by the time u apply. U can apply for a CS date by Dec 2006 and then take the date before May 20th ..or apply even earlier if dates are running out fast on the Step 2 CS Calendar.

Yes - applying again would be better than extending , if its July - coz most interviews occur in Oct-Nov-Dec , though offers may come early. At times, you may even be lucky to get a multiple entry visa when u apply for CS visa.

i have a query, i am just starting my prep for step 1 for the end of march 2007. i work full time in uk, have most weekends off. how many hours do you think i should spend studying for mle to be adequately prepared 4 march?, i'll be grateful for some advice. i am studying the kaplan notes. intelligence and absorptive capacity:average :)

Answers:

1. Full time worker in UK - you gonna need all the hours u can find and keep revising from time to time to keep up with urself :-) its best to structure ur entire timetable, follow it diligently and make small notes to revise stuff u really find tough. 1-2 hours a day and then as much as u can do on weekends.

2. Apply for the 2008 match too - based on the scores u get..a great score from fresh med-graduates always find some interviews somewhere. Even for 2008 match, if you can Fix some USCE before sending in the application, u can mention it. If things dont work out..work on the USCE and get going thru 2009 :-) g/l

yes a new token each year ...so for 2008 match, ERAS tokens from July /August 2007

All your info and CAF and SOPs will be wiped out for the new match application...so save those SOPs onto ur hard disk .and alo save the entire CAF (use view CAF function on ERAS and then 'select all' ..then copy -paste onto a word document...) . so that u can copy paste most of the CAF content for next years match application..

I dont have a personal experience with them..but someone on google anwers once mentioned that they charged a lot and were not that reliable for the residency search atleast - am not so sure bout externships and all. GMCgroup.org helps give an externship in almost any city u want..(they say so)

hello. I'm ECFMG certified and I plan to take step 3 by 3rd of Jan, is there a chance for me to get an H1B visa even after the programs have submitted their rank order list(feb 21)? I'll probably have the result by 2nd week of Mar granting that it takes 8 weeks before fmb release the result. Thank you so much. Yes - the rank order list does not contain any committments for the type of visa they are offering. So if you get ur Step 3 score fast enough before you sign the match contract (i.e. after the match day in mid-march) ..still a good chance if the program is open to H1b visas :-)

Hello Mr. IMG - u can apply for fellowship directly to some places once you complete a PG (Post Graduate MD) in your country, sinced all fellowships will need some MD training before hand -most of them will need a American Residency. You may apply to fellowships from the second year of ur residency

Yes - its possible with dedicated daily study :-) Apply for the CS now itself - u get a one-year permit. since dates go out pretty fast, its good to book early.

Yes, your timetable seems good, especially with that one month buffer for Step 2 CK, which by probabilities - you will need ;-). Considering 4 weeks of score reporting time, July end should be the deadline for taking the Step 2 CK in your case.

Yes 3 weeks are great for Step 2 CS - in fact you won't need more than 2 weeks of dedicated study for Step 2 CS.

Couple match - shall be putting up a post sometime ...when it does, it will show up on the "What's new" section.

To Rakesh: Observerships / externships are not always formal programs, rather arranged informally if a physician is willing to allow you to work under him. So the first obstacle to get by is finding a faculty who will be willing - secondly, some programs may have strict policies on allowing foreign grads to observe / extern at their places. The best thing to do is write emails / call programs - especially the ones listed on the USCE blog at www.USMLEtoMD.com/USCE. Other strategy is speak to your resident friends and have them talk to attendings that might consider. Finally, there are agencies like FMG portal which can give you paid experiences ( about 400 to 500$ per week !)

years-since-graduation criteria varies from program to program - you will find some need less than 3 years while other need 5 or even 10. Generally, the good programs have found to prefer 5 and less - but high scores and some US clinical experience or research publications often give older guys an edge over the fresh candidates

IF you keep your transcripts on 'automatic transmission' on your ERAS application, yes the directors will be notified of your result even before you get them. Yes you can apply with a single score alone - Read more about applying on a single USMLE score here


Answer 2:

Scores are king - do not compromise at any cost. The only scenario I would advise you to consider 2008 is if you are about to break the 5-years after medical school (coz some programs have that as a screening criteria)

Now my case:
I have given step1, scored 99 in it. will give CK in August ( i know it is late but considering graduated this january and came here immediately on F1, that is all I could do!) My CS results will come after October 10th. I managed with an observerrship meanwhile here.
my Q here is will I be considered by programs at all to call for the interviews as by then all the interview calls will have been despatched according to your blog.
your has put me in doubt if the 3500$ I need to spend (plus the mental agony) will be a waste for this fultile attempt. please do respond

With ur good step 1 score, observership and fresh-grad status, i would advise you to apply on sept 1st - but spend time this july calling programs to see if they will consider an application with a 99 step 1 and step 2 CK results coming up at Sept. end - the ones that respond positive - surely apply !

Yes - ur step 2 Ck scores cud arrive on time too - set ur transcript on automatic transmission.

Internal medicine is getting tougher by the year and 83 may be on the lower side - extra activities like US clinical experience, US lors and some US clinical research will help a lot - be prepared for the possibility that 2009 match may be a better chance if you can add all that to ur resume

That would be late for many programs and even most of the others would like see both Step 1 and Step 2 CK scores by that time - but you can always call potential programs then and see if they are still reviewing and taking more applications and apply to those. Anyways you can decide a limit on the dollars you wanna spend and apply within that to programs.

2009 Match may be a better idea if you can have all Step 1, 2Ck, 2CS and 3scores in hand and set up US Clinical experience or research

Most Interview Invitations will be over and out by November end - unless there are interview cancellations at other places. Additionally, you may not be ECFMG certified before mid-february and

2009 is a better idea. Take your time for the USMLE Steps and get good scores, and start off with US clinical / research experiences before you apply in Sept. 2008 for the 2009 Match - thats a better bet

Yes...step 2 CK is on target and so you will have both Step 1 and Step 2 CK ready while applying, which is good.

Before or along with Step 3 prep, try to have some US clinical experience (USCE) too - for IMGs, I like to say USCE is the new Step 3!
