LAST FEW MONTHS BEFORE MATCH: SOME TIPS

By Mid-January, a major bulk of the residency interviews will be over. A few places may still trickle out a few interviews, but things are pretty much wrapping up for the final 'Rank-order-list' preparation meetings of the resident selection committees across the participating programs.

This post applies more to the candidates who have had interviews and now start the waiting game. Would not apply to the fortunate ones who pre-matched either into the programs they wanted or did not mind getting into, besides the unfortunate ones who did not land any interviews this time.

Nothing is sure about the match - ten interviews and more in a specialty may have a larger mathematical possibility, whereas a single great interview can still be game ! As for the prematches part, we did see in the 2005-2006 season how even a prematch is not a surety (Forest Park Residency Program shut down for financial reasons before the match - even after giving out prematches to IMGs!) ...not that I wanna scare anyone, just mentioned some facts.

"Breathe. Let go. And remind yourself that this very moment is the only one you know you have for sure." - that was not me, it was Ophrah Winfrey who said so. She is surely right about the surety part, but we are not ready for 'let go' part yet...Not until the Rank-order-lists are finally submitted by the programs he he ;-)

Here are a few things to do to improve chances for those who have had interviews (especially the ones with few interviews):

1. Have you updated the programs about what you are doing lately ? It's a good time to right that with the new year wishes, if you have not already :-)

2. How about proposing to all your interviewed programs that you would not mind observing in the program for 2 to 4 weeks prior to the start date ?

3. If you have had interviews, starting any new US Clinical Experience or Research work now could still be of some help in improving your rating before the program directors submit their rank order lists.

4. If you are particularly very very very very keen on some program you interviewed at, you may ask for a second visit yourself (coz not all programs have a habit of giving out second-looks on their own)

5. A resident told me how he send personal paper greeting cards by post to all the programs that he had interviewed at, before February, thanking them again for the interview and expressing how badly he wanted to be with them - I leave that open to debate and personal choices, not commenting on that ....just mentioning !

Hello Digital Doc,

This has been issue that I remain unsure about.I have had a few interviews. I have written thank you letters already , back in November. My interviews were early, from September to Middle October, six of them. Each of the PD's have already personally responded to the thank you notes. One PD has hinted strongly at a prematch offer to come in January-not yet seen.
Now, I wonder, to show active interest now, nearly 2+ months since last interviews, SHOULD I NOW WRITE A 1-2 LINE NOTE, EXPRESSING MY CONTINUED INTEREST, OR IS THAT TOO MUCH..SHOULD I JUST WAIT FOR THE MATCH.

What do you think?
Thanks for your comment!

Sorry, Digital Doc the last post re response now in Jan to PD's, my name is Dr. Jackson

Yes - its always a good idea to keep the communication open since it has been a long time since your last email. Write to the PD enquiring if a possibility of a prematch exists, and that, if not, you will be ranking them in the top __ programs. Not bad to give updates about what you been doing so far, to all programs, and that you hope to match into their programs.

Hi DIGITAL dOC,

I FOLLOWED YOUR ADVICE..
1 PLACE SAID.. They could not offer a prematch, But they were QUOTE - 'VERY IMPRESSED, YOU ARE HIGHLY REGARDED'- I did an observership with them.

Another said they would match me highly , then they wrote again in a week saying they are offering me a prematch.

Here is the funny part, I am an old IMG - 1992, so you can well imagine me being eager to accept the prematch, but my heart was really with the other place -- 'highly regarded'one.
They seem however to be unable to actually offer a prematch by policy , its a big university, and I wish I coulsd get them via the macth, but just too risky for me to wait and misss the match.

i have noted other people speaking on other forums of promises from great interviews which eventually don't produce a match.

So its a case of sure for unsure..

So thx for your great advice, do you agree with me... take the sure prematch - older IMG,, or wait with nerves of steel for match ..haha ?

Digitaldoc, MD said ... (January 31, 2007 9:39 AM) :

I would most probably go in for the prematch in your position, if that program isn't all that bad for two practical reasons. 1. I believe that if a program really wants you, they usually do more than a mere "you are highly regarded" reply to an email - I mean they are more proactive in making the "want" palpable to the candidate. 2. Secondly, some residents opined to me that rejecting a prematch at a not-so-bad program may reduce the mathematical match chances in that program - because it is very likely that someone else will accept that offer, making less positions available for the match. But then, u gotta decide based on how many interviews you have.

And of course, the granny's advice - probably better to marry someone decent who loves you, rather than wait for someone whom you love but is unsure about you..LOL

1992 you said ? wow - I would love to have you contribute your advice to Old IMGs - (if you wish, write to digitaldoc2002@gmail.com)

Hey Digital Doc,

Thanks for all your help..so here is my story to help others..

Since not everyone gets 99,99,99.

MY STATS:

IMG Geaduate 1992
No Post graduate studies since
Attempted usmle step 2 1994 - FAILED
Worked at home since.

JULY 2005 DID STEP 1 - 88 /216
AUG 2005 DID STEP 2 CS - PASSED
FEB 2006 DID STEP 2 CK - 94/232
JUNE 2006 DID STEP 3 - 79 / (MESSED UP 2 CASES)

APPLIED 7 AM SEP 1, 2006
FM PROGRAMS 58
IM PROGRAMS 48
O+G PROGRAMS 18

NO GREEN CARD.. ALIEN

SO NEGATIVES...
NO GREEN CARD
FAILED EXAM 1 TIME
'OLD IMG' - 1992

POSITIVES
EARLY APPLICATION
STEP 3 DONE
OBSERVERSHIP DONE SEP -OCT 2006 USA AT UNIVERSITY PROGRAM

SUBSEQUENT TO OBSERVERSHIP RECEIVED THREE COMMENDATIONS FROM THE OBSERVERSHIP-UNIVERSITY PROGRAM DIRECTOR AND ASSISTANT PROGRAM DIRECTOR REGARDING BEING A VALUED AND REGARDED CANDIDATE -THEY INDICATED SEVERAL TIMES VERBALLY AND WRITTEN THEY WERE IMPRESSED.

I RECEIVED 5 INTERVIEWS IN USA AND I APPLIED TO CANADA TOO AND GOT 5 FM INTERVIEWS.

IN USA GOT 3 INTERVIEWS IM
2 INTERVIEWS OG
NO FM..CAN'T BELIEVE THAT.

NOTABLY, THE INTERVIEW OFFER WITH THE OBSERVERSHIP RELATED UNIVERSITY ABOVE WAS NOT EVEN SENT TILL I FINISHED THE OBSERVERSHIP IN NOVEMBER - HENCE THEY DID NOT REALLY CARE MUCH ABOUT MY APPLICATION OR WERE NOT IMPRESSED TILL THEY SPENT TIME WITH ME .

NOTABLY, FIRST INTERVIEW OFFER CAME SEP 5, MY BIRTHDAY HAHAHA, 4 DAYS AFTER APPLYING, THE REST CAME OCT TO EARLY NOV.

FIRST INTERVIEW SEP 29

AT THE INTERVIEW, I WAS THE ONLY APPLICANT THEN BEING INTERVIEWED THAT DAY.THIS WAS A PLUS!!!

THEY TOLD ME THEN AND THERE THEY WERE VERY IMPRESSED AND DO EXPECT TO EITHER MATCH ME AND/OR PREMATCH ME

THIS WAS AMAZING TO ME CONSIDERING MY NEGATIVES.

NOV-JAN
I TRIED TO TWEAK INTEREST FROM THE OTHER 4 INTERVIEW PLACES - INCLUDING THE OBSERVERSHIP LOCATION

THE BEST RESPONSE WAS AS I TOLD YOU FORM THE PLACE I INTERVIEWED AT--THEY SAID..'WE WERE VERY IMPRESED BUT CAN'T PREMATCH ME'

THEN I WROTE THEM AND TOLD THEM I PREMATCHED AND THEY (PD)
WROTE BACK ACTUALLY ASKING ME TO STAY IN THE MATCH AS THEY WOULD BE RANKING ME!

HOWEVER, EVEN THOUGH I DO PREFER THEM I JUST COULD NOT TAKE THE CHANCE.

SO I CONFIRMED WITH THE PREMATCH.
THEN I CANCELLED THE FM INTERVIEWS IN CANADA.

SO I AM FINALLY IN THE USA SYSTEM.

PLUS, BOTH THE OBSERVERSHIP AND THE PREMATCH OFFER H1b VISAS..

I HOPE THIS INSPIRES OTHER IMG'S ESPECIALLY 'OLD' ONES THAT THEY CAN STILL PACKAGE THEMSELVES TO BE ATTRACTIVE TO PROGRAMS AND OUTPERFORM OTHER APPLICANTS..

MAIN ASSETS
1. PRAYER ALWAYS WORKS

2. COMPLETE ALL USMLE STEPS 1,2,3 BEFORE SEP 1 AND APPLY EARLY.

3. DO AN OBSERVERSHIP.

4. TRY TO HAVE AN EXCELLENT INTERVIEW -MOST IMPORTANT . LEARN ABOUT PROGRAM AND MOCK INTERVIEW YOURSELF .. BE ATTENTIVE AND BRIGHT!

5. PRAYER AND DEDICATION TO YOUR GOAL NO MATTER HOW YOUR- OUR 'NEGATIVES' MAY LOOK

6. AND IT HELPS TO HAVE A SUPPORTIVE SPOUSE! - I DID.

SO HOPEFULLY ALL THOSE OUT THERE WHO READ THIS 'BELIEVE' IN THEMSELVES, THAT THEY CAN DO IT TOO.!!

hello - thanks for the great write up - your story is indeed most inspiring to all OLD IMGs out there reading this blog - thanks and congrats :-)

Dear Jackson & Digital Doc
And I thought I was a very old IMG!I am Indian, completed internship in 1999, did PG in IM, working as senior resident now.Step 2 99, CS results awaited & Step 1 in Aug. I wish to apply for Surgery! Dont laugh! Do I stand a chance? Of course I will apply to IM & FM too.I always wanted to be a surgeon! Moreover how does one apply to Canada with USMLE scores? Have your laugh but please reply.

Digitaldoc, MD said ... (July 06, 2007 2:47 PM) :

Yes - you can still have a chance - apply to all prelim as well as categorical positions - but what your resume may lack is something to prove your commitment to surgery - in the form of US clinical experience in surgery or some surgical research ...that will certainly help you tons, besides explaining on your personal statement the switch from IM to Surgery.

Canada does not accept USMLE scores - the only other country to accept USMLE scores, in my knowledge, is New Zealand - and that too only Step 1 and Step 2 Ck within 5 years
