ECFMG CERTIFICATE : Basics & The Fine Points
First let's tackle a basic question. "How Does the ECFMG certificate actually look ?" Well guys, .simply check out mine : Click on the image below to enlarge.

Second basic Question : Do you need to be ECFMG certified to apply for ERAS or Residency Match ?
Technically speaking, No ! You don't need to be ECFMG certified to either apply to Residency Programs OR to register for the NRMP Residency Match . However, you do need to be ECFMG certified before Programs submit their Rank Order Lists , which happens by Mid-February of the Year of the Match. (i.e. if you are thinking 2008 Residency Match , you should aim to be ECFMG certified before the 2nd week of February 2008 ). And here are the words from the University of Alabama Internal medicine website:
"An ECFMG certificate is not required at the time of application, but in order for IMG’s to be considered for an interview, they must have completed USMLE Steps I and II and the CSA exam when submitting their applications (or the scores must be available prior to a scheduled interview). IMG’s who are interviewed for this program MUST provide documentation (an ECFMG letter of certification or an ECFMG certificate) to our office PRIOR TO February 15, 2006"
Now, that was 'Technically'. These days, with increasing competetion, some programs may even wanna see you ECFMG certified before they can review your application - which means, you will need to call the programs u intend to apply to and find out the policy for the year...
When do you get the ECFMG Certificate ?
The Certificate usually arrives within 2 weeks after you have received ALL THREE SCORES (Step 1, Step 2 CK and Step 2 CS), PROVIDED all your medical credentials have been verified with your medical school by then. Thus, if the last exam you took was the Step 2 CK, the Certificate will arrive within two weeks of the score reporting date for Step 2 CK on OASIS - unless ofcourse, you medical school has not yet verified your credentials with ECFMG. If not, then it will arrive 2 weeks after your credentials have been verified - *Duh* ;-)
Do you need to "apply" for it ?
No, there is no need to apply -ECFMG will send it you on their own , once you are qualified for the certificate.
Anyways, there are some more things I would like you to know about the certificate.
1. For those of you who will have applied to programs through ERAS before being "certified", its good to know that even though the "physical paper ECFMG certificate" arrives to you after two weeks and all - the programs that you apply to will receive your certification status electronically much before you, as an updated transcript. This fact may be useful for those few of you who may happen to play a tight rope-walk with time during the 2007 match (or any other match). Indeed some programs may convey to you that unless you are certified before they submit their candidate rank order lists in mid-february, they will not rank you.
2. Step 3 Application needs you to be ECFMG certified - they need the date of your ECFMG certification. Hence you can only apply for Step 3 AFTER you receive the Paper Ceritificate by mail.
Hmmmmmm..just remembered something else - Some pals told me that the day you get the ECFMG certificate, you can legally label yourself "MD" in the USA - I am not totally sure of how qualified that statement is , coz even the fact that you graduated from a med school makes you an "MD" here. Read more about this IMGs calling themselves MD issue officially on the American Medical Association Website.

hi digitaldoc, thanks a lot for making things easier for us, you are in our prayers now.i have a request, please send me a copy of your dean's letter & one LOR, as my college authorities arent used to write such documents. Actually,i wanted to ask something regarding certification..is there any impact on the number of H1 calls if you are not certified by the time you apply i.e. sep1..? what if you get certified by oct end?

Some community programs with excess applicants may use it as a screening criteria - but dont let that bother u ...Oct is good enough too :-)

i am in exactly the same tightrope situation you were talking about in your post- my score got updated this week, just 2 days before sep 1st.
im planning to apply to programs as soon as possible so i wanted to know- if i click on 'autotransmit score' on the usmle transcript- will my ecfmg certificate alongwith my step 2 ck score go to them at the same time?
also, i should be getting my certificate in 2 weeks, so iwouldnt know the date of certification. there is a section on caf asking if youre certified and month/year. since my credentials are verified and im likely
to get my certi in sept of this year, can i go ahead and put sept/2006? or should i call em and find out what my certification date is?
please do help out.. im at my wits end!!

Answer 1 : Yeah ..they will go at the same time...
Answer 2 : Sept is safe bet to mention certification date as ! all that PDs care is if u will be certified before Feb 2007 or no :-) - the exact certificate date will matter for Step 3 application. G/l -

Hi - you dont need to have all scores ready to apply on Sept. 1 - Step 1 is enuf...but do plan to take other steps as fast as u can without compromising scores :-)

Digital Doc, I have a question. Most residency programs websites say in their "how to apply section" that you need to be ECFMG certified.
Only, let say, 1 out of 20 programs I have seen clarify that the ECFMG is only needed prior to match list submission.
So what is the deal? Do I or dont I need the ECFMG certificate to apply? Will they read my application if i only have my Step 1 and 2 scores?
Im kind of dissapointed. Even in the First Aid for step 1 they say in the IMG timeline for Application that the ECFMG certification process (finish steps, etc) should be done by December.
I feel that following that advice I am out of many programs.
Am I wrong? Do they mean something different when they say you need to be certified to apply? Is it that I am looking in non-fmg friendly programs?

Well, its a given that the ideal to have Step1, Step 2 CK and Step 2 CS scores ready by the time you apply on Sept 1, then the only component left for ECFMG certification is medical school verification.
Most programs want you to be ECFMG certified by the time of submitting ROL to be on the safe side (technically not needed for the match per se, but surely before residenc starts), if your application really impresses them, they could even give you time until before residency starts for the certificartion to come by, but yes, usually they would like to see the three scores in hand before ranking you.
Many programs may want that certification by application deadline - which is NOT Sept 1, but which changes from program to program and usually hangs around Dec 1st week. The rush to apply by Sept 1st is to capture early interview slots, and its only natural that first-come-first-serve will work and candidates with complete applications will get the interview calls
Lets check out some programs
1. Marshfield clinic IM program: "You do not need to have an ECFMG Certificate to apply. Application Deadline is January 1st, however, as we tend to fill interview openings early I would suggest you have your application submitted by November 1st if not earlier"
2. Duke Family Medicine Prog: "Have current ECFMG certification. Must be ECFMG certificated no later than the start date of the program"
3. Morehouse Program:
ECFMG Certification must be final prior to [or on] Match Day
4. St. Elizabeth IM program:
"You will not need to have ECFMG certification when applying, but will need it to be qualified for the match."
In all, ideal to be ECFMG certified by Sept 1st, but I would not sweat too much about it, what matters first is good time to study to get good scores !
Univ of Kentucky IM Prog: We do not have minimum USMLE score requirements, but you must be ECFMG certified by June 30 of the year you are wishing to start residency.

Thank you for all the wonderful work you are doing... I am a little stressed regarding my ecfmg ceritification. I have already completed all my steps 1,2 & cs. However i got a rude shock last week when ecfmg said they needed to verify my medical diploma inspite of me having asked them a zillion times before... I do not know how long it will take ...will this afect mt chances ...i would really appreciate if you could help me out
You should be on the case of medical school verfication like a cat on a rat ! Find out if your medical school has not done its job. Some medical schools take extra $$$ to get that favor done, so find out from your medical school office. Also the next time you call up ECFMG, tell them that your school sent it to ECFMG long back and urge them to check once with with the documents department to verify.
