APPLYING WITH SINGLE OR TWO USMLE SCORES


Each year, many IMGs are faced with the scenario of taking a shot at Residency Applications without having all USMLE scores in hand - like, maybe just Step 1 , or just Step 2 CS, or any two scores ...

If we considered having Step 1 + Step 2 CK + Step 2 CS scores as 'complete', then the most 'incomplete scores' combination happens to be Step 1 + Step 2 CK - but this combination is still much better than other possible combinations. Been hearing of Step 1 + Step 2 CS very very often these days, thanks to lack of CS dates which makes people take the CS as soon as possible.

Supposing you decided to go ahead and apply with incomplete scores, lets address some poignant issues in this context :

a. Yes, Interviews are still possible applying with Step 1 scores alone, though over the years the probability has been getting lesser. Yet, to maximize chances, you gotta send in your applications early AND the Step 1 Score should be impressive enough to garner enough confidence in the program director about your subsequent performance. It's should be tacitly understood that the number of residency interview calls you will get will be much less than if you had applied with both USMLE Step 1 and Step 2 CK scores.

b. Remember that on your ERAS application, you are allowed to specify the expected month of remaining USMLE Steps, in case you have not taken them all by application time. Don't let that be too late - remember the 'ECGMG certified' status must be completed before early February. So if the program believes that you will not be certified until then, it will put your application on the backburner.

c. The next natural question : "Should I apply early in September with my Step 1 score or should I wait till I get my step 2 CK result later ? "

Now, this was a question someone shot at me over email and I am copy-pasting the reply below :

While there is no sure-shot answer to this question, lets consider some alternatives =

a. Write to/call all programs u wanna to apply to and see if they will consider ur application with a Step 1 score alone and that you will be taking other exams soon. Definitely go ahead and apply to the ones who reply positively - the ones that say 'No' or Don't reply, apply after u take ur step 2 CK - and keep providing updates to those programs about ur exams, scores, observerships, etc.

OR

2. Another strategy is to apply to low-traffic, moderately-ranked or low-ranked programs with whatever scores you have - and the others after all scores come in

OR

3. Go ahead and apply to all programs and then keep on sending updates as when u take ur exams, get ur scores, etc - the only problem i see in this approach is over-spending.
In any case, you will benefit most by personally calling / emailing programs :-) I know it ain't a perfect solution but I do hope this gives you some direction"



Choosing your alternative is a personal choice incumbent on factors like how much $$ you willing to risk, or if you simply have to match this year instead of adding more years to your "Old IMG" status, or considering whether you are rather better off for the next year's match and use the extra time to complete even Step 3, get some US Clinical experience & better LORs or whether you got contacts that can pull you an Interview at a program, etc. So ...choose your 'trade-off's guys and decide.




And I shall now go back to a hot cup of tea waiting for me .... LOL

ECFMG certification is not needed while applying thru ERAS and officially NOT needed even for registering for the match - but certainly needed before starting a residency. But Program Directors want to see candidates ECFMG certified before they submit their Rank Order Lists in Mid-February. Thus, you can still register for the Match without certification, but if you are not certified before ROL submission (i.e. dont complete passing Step 1, Step 2 CK and CS and credential verification) , you are out of the match coz programs wont enlist u ..

Yeah both the options are possible but the way I see it is better use one year to boost your application by completing all the steps and getting some USCE,because that might just land you in a good place.I mean where you do it is also important isnt it?

Thats true and thanks for the comment- Its also true that a lot of candidates, apprehensive about the increasing competition, are also happy just to 'get in somewhere' and get on with life :-) As i said, the choice is based on personal values...many argue that no matter where you do it, payscales wont differ much.

I applied to many programs with step 1 score(no interview calls yet)and have just got my step 2 score too; m taking CS in the end of October.
Should i send emails to the programs regarding my step 2 score? wouldnt my application still be considered incomplete- without CS result?

Yeah - do send them an email, though they will be receiving the scores before you do via electronic transcripts. It does make atleast some program to give ur application another look and also indicates continued interest in the program from ur side

Digidoc, I was the one who asked you this question over the email . whether I should apply before or after the CK score report. I did my homework, called the programs and applied wisely to 25 programs with step 1 score. Lo Behold.. I got 5 interviews. I applied to another 25 programs after I got my step 2 report a week back. I now have another two interviews. Total of 7 with cs report due in December. So everybody.. do your research.. call programs... apply wisely ..rather than blindly applying to tons of programs. It will cause the PD's less confusion and save us some moneyy ;)

Hi Digital doc,
You're doin' one heck of a job with this blog! i bet you got more than 100,000 people lookin' at it everyday.
Anyway, I needed your advice concernin' the situation i'm facin'. I got sick a couple of days before my step 1 test, but i entered anyway, and for my surprise, i passed but with a score of only 79! I intend on applying for the '08 match, givin' me enough time to do both the step 2 exams. what should i do to improve my chances?

I am sorry to hear about ur sickness..thats a bummer. Anyways, youare doing the right thing by thinking ahead..a better score obviously on the other steps, good US clinical experience for 3 to 6 months(which will also give you US LORs) and applying very much on time to prorams with individualized Personal Statements...and highly targetted applying by calling up programs around August and asking them if they will consider ur application or not.. :-) g/l u gotta pretty much try for a very good Step 2 CK score

i have already have my step 1 and step 2ck, i took my cs on mid nov and the score report date is around end of jan. I have done some interview but no one give me prematch.my step1 and 2 are 82/82. fresh grad. have US experience and LORS, i know somebody with same kind of creditinal but do have several prematch call, so i am wondering is that becasue of my cs result is not released yet so that's why they do offer me prematch? and also since the nrmp quota date is also by the end of jan. when i get me CS result,can i still get prematch even after the send the quota to nrmp? thanks a lot.

Sure you can - the quota is usually predetermined - the Jan is only the deadline to convey it for the year. Yes, usually program directors would like to see an ECFMG certified candidate before they offer a prematch - but not always the case.

I have a quick question... I have taken my USMLE STEP 1 and passed. Now I want to make the 2008 match..Do I have to take the CS to even enter the match or can i settle with STEP 2 CK+ STEP 1 and then take the CS after I have sent in my application. I am asking this because the Step 2 CS test date availibity is scarce. So the question is can I get interviews with STEP 1 + STEP 2 CK and then hand the programs my CS status later?

Sure you can apply with step 1 and step 2 Ck score alone..and even register for the match. What matters is getting the results before the program directors (at the programs you get interviews at) see your step 2 cs pass before they submit their rank order lists for the match year - which would be mid-feb 2008 for the 2008 match

Keep your transcript of manual transmission mode, apply with both scores - if step 2 CK scores are good, you will get calls..

Answer2: haste has always made waste - dont compromise your score, but take it too long either. If you are confident of crossing the 90s in ur 'haste' , then maybe it is worth it, otherwise a few weeks more is advisable

I have question...done with step 1(not great score),step 2 CS but got 74 on step 2 CK .. planning for ck again in late dec -early jan . Have 1 LOR from home country (done residency back in home country) and 2 from US(internal medicine)should i apply now ?? what are my chances in getting residency this year??

Apply only to programs that you can call up and confirm that they are still accepting applications and will review your application with those scores - if not, its better to save your $$$ and apply next year with all passing scores
